It can, if URLs change without redirects or if content gets thinner in the rebuild — those are the two most common causes of ranking drops after a redesign. Careful URL mapping and 301 redirects for every existing indexed page is the main defense against that.

Do you keep the same URLs?add

Where possible, yes — it's the simplest way to protect rankings. When URLs do need to change (a genuinely better structure, a platform migration that forces it), every old URL gets a 301 redirect to its new equivalent.

Can you migrate off WordPress?add

Yes — content gets extracted from the WordPress database or export, and rebuilt on the new stack with the same URL structure and metadata carried across.
